CCHC Community Outreach department is a corner stone of CCHC’s mission to serve the uninsured and low-income families from the underserved communities in Metro Los Angeles Area and San Fernando Valley Region focus on health promotion and community intervention. The Outreach Team works directly in the clinical and community-based settings through inter-department coordination and a collaborative network of community, school and faith-based organizations, government agencies, and health care providers. The team provides individuals and their families access to primary and preventive care including health education, insurance enrollment, screenings and referrals in all sites and increase community involvement and active participation of community partners in CCHC outreach and education program.

Our Outreach department organizes and hosts various events and provides support services throughout the year in an effort to connect our community to care.

Project In-Reach and Patient Re-Call for Access to Care:

Connect with patient through in-reach and project re-call activities to update and promote clinic services to patients and set their telehealth, telephonic and in-person clinic visit.

Health Promotion and Awareness Campaign:Conduct health awareness and promotion campaign through a one-on-one encounter connecting with patients via telephone providing health education and information on a variety of topics including COVID-19, Women’s Health, Family Planning, Diabetes and Hypertension including Health Coverage and Enrollment process for Covered CA, Medi-Cal, Medicare, MHLA, CHDP, Family PACT programs.

Community Engagement and Program Partnership:Establish collaborative partnership, conduct program orientation, tabling information, consultation and focus group discussion through on- site activities, phone or virtual meetings to community, school and faith-based organizations including government to assist the underserved populations obtain primary care, health education and information, medical referrals and guidance in using healthcare system. Expand and formalize program partnership through MOU’s.

Outreach Support Services and Annual Events:

  • National Health Center Week – “Stakeholders Appreciation Day”
  • Back-to-School Children’s Appreciation Day
  • Annual Toy Giveaway for Children
  • Clothing Campaign – Jackets, Sweaters and Blankets Distribution for Day Labor Workers
  • Refugee Assistance Collaborations for Afghanistan and Ukrainian Families
  • Breast and Cervical Cancer Screening Collaborations
  • COVID-19 Testing and Vaccination Day

CCHC Community Outreach and Community Intervention activities are very instrumental tools and proven effective to update and promote clinic services to patients. CCHC will continue its in-reach and outreach activities and health promotion campaign to reach out help those in need find our clinic and utilize our services specifically those low income and underserved individual in Metro Los Angeles area and San Fernando Valley region. The outreach team will also maintain its collaborative relationship to community partners which served as strategic channels and communication to patients and community partners towards meeting CCHC objectives.

S.H.A.P.E.

Sexual Health And Family Planning Education (S.H.A.P.E.) is a CCHC developed campaign funded by the federal Title X family planning program. S.H.A.P.E. is intended to engage the community about family planning and other related reproductive health issues. We partner with middle schools, high schools, community colleges and other community-based organizations that provide services to individuals of all ages.

S.H.A.P.E. offers individuals information and skills regarding:

  • Access to Clinic Services
  • Contraception/Birth Control
  • Sexually Transmitted Infections (STIs)
  • Healthy Relationships
  • Intimacy & Values
  • Teen Pregnancy Prevention
  • Communication
  • And more!

S.H.A.P.E. is aligned with California Education Codes 51930-51939 and is intended to help fulfill some of the Health Education Content Standards for California public schools.
