Determining Eligibility for Health Insurance Coverage
Eligibility is determined based on household size, earned income, and residency status. The following documents are required to determine your eligibility:
- Identification
- Proof of Address
- Proof of Income

Below is additional information about Medi-Cal, Covered California, My Health LA, and the Sliding Fee Discount Program.
Medi-Cal
The California Medical Assistance Program or Medi-Cal is California’s Medicaid program serving low-income individuals, including families, seniors, persons with disabilities, children in foster care, pregnant women, and childless adults with incomes below 138% of the federal poverty level. Medi-Cal enrollment is open year-round.
Medi-Cal Expansion
Beginning January 1, 2024, a new law in California will expand full-scope Medi-Cal to all low-income adults, REGARDLESS OF IMMIGRATION STATUS.
- Full Scope Medi-Cal covers a lot more than just emergency services, it is full medical coverage that provides medical, dental, mental health, vision, specialty care and more.
- Full Scope Medi-Cal benefits (except for nursing homes or mental health institution care) will not hurt an individual’s immigration status.
- If you are already enrolled into Restricted/Emergency Medi-Cal, your benefits will automatically transition to full-scope benefits, no action required. If you are not enrolled, you can enroll into Restricted/Emergency benefits now to ensure a smooth transition on January 1, 2024.
- Due to this expansion, My Health Los Angeles (MHLA) will sunset on January 31, 2024.
Medi-Cal Redeterminations
In January 2020, the U.S Department of Health and Human Services (HHS) declared a Public Health Emergency (PHE) in response to the outbreak of Covid-19. As a result, Medi-Cal beneficiaries received continuous coverage without having to submit their annual redetermination. Starting April 1, 2023, Medi-Cal will resume its normal redetermination process.
Medi-Cal Renewal Form
The Department of Public Social Services (DPSS) will mail you a letter to notify you if you will need to renew your Medi-Cal or if it will be renewed automatically. If you receive a renewal form, CCHC can help you submit your renewal by mail or online, call us at 818.630.2296 to schedule your renewal appointment. Sliding Fee Discount Program
Our Sliding Fee Discount Program is available to all eligible patients who meet the eligibility guidelines. The eligibility requirements are based on the federal poverty guidelines published annually by the U.S. Department of Health and Human Services and are subject to change. The discounts for the program are based on the patient’s family size and household income.

Covered California
Covered CA is the state’s insurance marketplace. Covered CA offers affordable and low-cost health insurance options. Based on your income, you (and your family) may qualify for assistance through Covered CA.
Special Enrollment: February 1- October 31
Open Enrollment: November 1- January 31
You must have a qualifying life event during the Special Enrollment period. If 60 days pass from the event and you do not enroll into a plan, you will have to wait until the next Open Enrollment period.

My Health LA
My Health LA is a no-cost health care program for people who live in Los Angeles County and meet the eligibility requirements. It is a free health care program for individuals and families who do not have, and cannot get, health insurance.
