Our on-site pharmacies primarily serve CCHC patients to ensure integrated care, but you can ask about transferring prescriptions in certain cases.

Yes. We can arrange a transfer from your current pharmacy to CCHC’s pharmacy for most medications.
Do you offer automatic refills?
We offer refill synchronization programs so multiple medications can be picked up on the same date each month.
