Sliding Fee Discount Program
We provide a diverse range of services under the Sliding Fee Discount Program. This program is accessible to all qualifying patients and is determined by your annual household income and family size, following the federal poverty guidelines.

Good Faith Estimate
As part of the No Surprises Billing Act, uninsured and self-pay patients are eligible to receive an estimate of the possible changes for the appointment(s) that you schedule with CCHC. Your financial category will depend on your income and the number of people in your household. The estimate will contain additional information on how to determine your self-pay category.
