Comprehensive Community Health Centers, Inc. (CCHC) is a Federally Qualified Health Center (FQHC) and a California Non-Profit Public Benefit Corporation. CCHC operates four community health centers providing medical, dental, behavioral, and vision services to underserved people in metropolitan Los Angeles and the San Fernando Valley. CCHC health centers are located in the cities of Glendale, Eagle Rock, Highland Park, and North Hollywood.

CCHC is among the top community health center systems in Los Angeles County having grown in health service visits from 45,000 visits in 2004 to over 129,000 visits in 2015.

CCHC serves as a medical home to individuals and families across the life span, including the low-income and underserved. CCHC serves a large immigrant population including Latino, Armenian, and Asian populations. CCHC’s health care providers speak the languages of the community.

Adrian Furman

Adrian Furman, MD

Dr. Furman is a board certified pediatrician. He is a graduate of Stanford University, received his medical degree from Vanderbilt School of Medicine in 2012, and completed his internship and residency in Social Pediatrics at the Children’s Hospital at Montefiore in Bronx, NY. Proficient in medical Spanish, he has provided volunteer outreach services in Guatemala and Nicaragua. Dr. Furman is a former commercial fisherman from Bristol Bay, Alaska who enjoys spending time outdoors, including weekly hikes in Griffith Park with his dog Bella.

Adriana Arredondo

Adriana Arredondo, FNP

Ms. Arredondo received her medical degree from California State University, Long Beach in November 2011. Since, she has practiced at notable clinics including: Salud Digna Clinic, Florence-Hooper Family Medical Clinic, and Dr. Mayer’s Women’s Clinic. Ms. Arredondo has a unique combination of strong practitioner/nursing skills and strives to provide the best health care possible. She believes in building a long lasting and trusting relationship with patients and customers.  She seeks to acquire a comprehensive evaluation of every patient by taking time to listen and understand their particular needs. Her true desire is to enhance a positive healing experience and confidence to those in need. She strives for quality care above all else. Ms. Arredondo is fluent in Spanish. She loves to play voleyball is a big Dodger fan!

Alice Yoo

Alice Yoo, DDS

Dr. Yoo received her dental degree from Ostrow School of Dentistry in 2013. She has a combination of experience at both private and community health clinics, including Rancho Smile Dental Group and John Wesley Community Health Institute. Dr. Yoo speaks English, Spanish and Korean. In her free time, she enjoys yoga, cooking, and hiking with her husband and dog.

Alicia Garrido

Alicia Garrido, NP

Ms. Garrido received her medical degree from Harbor UCLA Women’s Health Center and has previously practiced at San Judas Medical Group as well as Victory Medical group. She is fluent in Spanish. Ms. Garrido loves traveling and spending time with her friends, family, and her five grandchildren.

Ana Vega

Ana Vega, PA-C

Ms. Vega attended UCSD and earned her B.S. in Science. Ms. Vega received her medical degree from Western University of Health Sciences in Pomona, CA. Since, she has completed her rotations at various community clinics including East Valley Community Health Center and Velez Clinic. She has been working at CCHC since 2005 and is fluent in Spanish. Ana has also provided community services at AIDS project as well as UCSD Medical Center. She loves spending time with her kids and scrapbooking their exciting lives.

Anh Quyn Do

Anh Quyn Do, DDS

Dr. Do graduated from the University of Southern California Dental School and has been practicing dentistry for over 8 years. She has worked in private practice, pediatric dental clinics, and non-profit health centers where she focuses her practice in the importance of good oral hygiene and preventive care in the maintenance of oral health. Her practice philosophy is to provide quality care with minimally invasive treatments.  Dr. Do has also participated in dental outreach locally and internationally.

Ani Baroni

Ani Baroni, OD

Dr. Baroni received her medical degree from Nova Southeastern University College of Optometry in Fort Lauderdale, FL in 2008. She has been practicing at CCHC since August of 2008 and is fluent in Spanish, Armenian, and Farsi. She lives in Los Angeles with her husband and two children. When she has vacation time, Dr. Baroni loves traveling to the Caribbean with her family.

Ani K

Ani Kartounian, DMD

Dr. Kartounian received her degree from the Tufts School of Dental Medicine in Boston, MA. She is a general dentist and has been practicing dentistry since 2009 treating children and adults alike.  Dr. Kartounian is fluent in English and Armenian.  She enjoys being with her family and playing the piano in her free time.

Anna Baycher

Anna Baycher, OD

Anna received her education from Southern California College of Optometry and has been practicing since 2012 although she has been in the field since 2007. She is fluent in Spanish/Russian and enjoys poetry, reading, bowling, and skiing.

Ara Airapetian

Ara Airapetian, MD

Dr. Airapetian received his medical degree from Yerevan Medical Institute in Armenia in 1981 and has been practicing at CCHC since 2004. Dr. Airapetian is fluent in both Armenian and Russian. Interests include: Chess, tennis, art, and classical music.

Arakel Davtian

Arakel Davtian, MD

Dr. Davtian is Board Certified by the American Board of Psychiatry and Neurology. He received his medical degree from the School of Medicine in Yerevan, Armenia in 1983 and completed several internships and residencies at King Drew Medical Center. He previously held positions such as Assistant Professor of Clinical Psychiatry and is fluent in Armenian.

Arthur Manoukian

Arthur Manoukian, MD

Dr. Manoukian is a physician specializing in internal medicine and has previously worked for notable organizations including City of Los Angeles and Health Care Partners. Dr. Manoukian has been practicing for over 20 years and is fluent in Armenian and Russian.

Ms. Budagyan attained her medical degree from Charles Drew University of Medicine and Sciences in Los Angeles, CA in 2011. Prior to practicing at CCHC, she previously continued her medical experience while practicing at Clinica de Maternidad y Pediatrica in El Monte, CA. She is also fluent in Armenian.

Emma Avetisyan

Emma Avetisyan, NP

Emma is an Adult-Gerontology Primary Care Nurse Practitioner certified by the American Academy of Nurse Practitioners. She attained her Bachelor of Science in Nursing from Mount St. Mary’s College and continued her education by completing her Masters of Science in Nursing from California State University. She is fluent in Russian/Armenian and has worked with notable organizations including Kaiser Permanente and Glendale Memorial Hospital and Health Center among many others.

Graciela Leslie

Graciela Leslie, LCSW

Ms. Leslie received her Master of Social Work from the University of Southern California in 2005. Since, she has provided social and outreach services in both English and Spanish to diverse populations at many organizations including Didi Hirsch Community Mental Health Center, MOMS Orange County, and Hoag Hospital Community Health Programs.

Henry Barraza

Henry Barraza, PA-C

Mr. Barraza received his medical degree from Western University School of Health Sciences in 2002 and completed his clinical rotations in the areas of pediatrics, family practice, OBGYN, and neurology. Mr. Barraza previously practiced at Northeast Community Clinic as well as St. Johns Well Child and Family Center. He is fluent in Spanish. Mr. Barraza enjoys watching sports, traveling, and staying fit.

Illina Beron

Illina Beron, NP

Ms. Beron is a Board certified Nurse Practitioner. She received her Nurse Practitioner education in Women’s Health and Family Practice from Harbor-UCLA and has a Masters in Nursing from Drexel University in Pennsylvania.  Being raised by a family of physicians has inspired her to focus her career on medicine, specifically women’s health care. She successfully combined raising a family of seven children while pursuing a fulfilling career. To her, medicine is an art, a science and a community service. She is passionate about women’s health issues and empowers her patients to find their inner strength to focus on a meaningful difference in achieving their quality of life.  She is fluent in both English and Russian and versed in medical Spanish.

Irina Godes

Irina Godes, MD

Dr. Godes is Board Certified by the American Board of Pediatrics and received her medical degree from Second State Medical School in Moscow, Russia in 1979. She continued her education through residencies and externships at Martin Luther King Medical Center and Kaiser Permanente, and is also fluent in Russian. Dr. Godes loves to work and when she’s not working, she loves to dance and spend time with her six grandchildren.

John Cabrera

John Cabrera, MD

Dr. Cabrera attained his medical degree from Creighton University School of Medicine in Omaha, Nebraska in 1991 and completed his residency at Saint Francis Hospital of Evanston in Evanston, IL. His previous post-graduate experience includes having his own private practice as well as working at Arroyo Vista Family Health Center. He is licensed in both CA and IL, and is also fluent in Spanish. He enjoys spending time with his family.

John Matchett

John Matchett, DDS

Dr. Matchett received his dental degree from the University of California, San Francisco in 2010. His previous experience includes practicing at multiple clinics including: UCSF School of Dentistry Clinic, Community Dental Clinic at San Francisco, and Burbank Blvd. Dental Care.

John Ter-Zakarian

John Ter-Zakarian, MD

Dr. Ter-Zakarian received his medical degree from the School of Medicine in Yerevan, Armenia in 1976. He held his own practice for several years and is affiliated with the Queen of Angels Hospital in Hollywood, CA. He is fluent in Armenian, Russian, and Turkish.

Karen D

Karen DeClue, PA-C

Mrs. DeClue earned her undergraduate degree in biochemistry and human development from UC San Diego. She then went on to study medicine at Quinnipiac University in Hamden, CT where she earned her Masters of Health Science and Physician Assistant Degree. She is fluent in Spanish and has been practicing family medicine since 2009. While living in San Diego, Mrs. DeClue became a lifelong Chargers fan, and she still attends games every year. She also met her husband there, and they now have two beautiful daughters.

Kevin G

Kevin Galstyan, MD

Dr. Galstyan is a Diplomat of American Board of Obstetrics and Gynecology. He received his medical degree from Yerevan State Medical University in Yerevan, Armenia in 1996. He completed his residency in Obstetrics and Gynecology at Mt. Sinai Medical Center in Chicago, IL in 2006. Dr. Galstyan is a Professor at Midwestern University, Arizona College of Osteopathic Medicine and is fluent in Russian and Armenian. He enjoys traveling and hiking with his family.

L.Yanaga

Linda Yanaga, DDS

Dr. Yanaga received her dental degree from the University of Southern California in 1992. Her previous experience includes practicing at a private practice as well as St. John’s Well Child and Family Center and Los Angeles Christian Health Centers. Dr. Yanaga is fluent in Spanish. Dr. Yanaga was in the United States Army Reserve and has traveled to countries including Germany and Iraq.

Lisa Nguyen

Lisa Nguyen, DDS

Dr. Lisa is a Board Certified Pediatric Dentist and received her dental degree from New York University, College of Dentistry in 2008.  She continued her education through various residencies and has traveled to Honduras and Jamaica to help underserved children without access to dental care.  Dr. Lisa truly enjoys working with children and considers herself a big kid at heart.  Dr. Lisa is fluent in Vietnamese and Spanish.

M.Michael

Manar Michael, PA-C

Ms. Michael received her medical degree from the University of California in Davis, CA in 2004. Since, she has practiced at various clinics and hospitals including: Pacifica Hospital, Beverly Hospital, and Golden Valley Health Centers. She is also fluent in Arabic. Ms. Michael likes to serve her family. She enjoys being out in the open especially at the San Diego beaches and Palos Verdes.

N. Haroian

Nairi Haroian, DDS

Dr. Nairi Haroian earned her Doctorate of Dental Surgery in 2011 from Ostrow School of Dentistry of USC. Dr. Haroian believes it is through efficient dental practices, dedication and commitment that she will help achieve improved oral health. Continuing education is important to Dr. Haroian. She stays current with her education in an effort to provide the community with new treatment techniques and new materials. Dr. Haroian has a vivid passion for serving communities with dental needs. Outside of the dental office, Dr. Haroian enjoys hiking, traveling, and spending time with her family. Dr. Haroian also enjoys learning about new places, languages, and ideas from all of her patients. Dr. Haroian is fluent in English and Armenian.

N. Neagos

Negoita Neagos, PA-C

Dr. Neagos received both his medical degrees from the School of Medicine and Pharmacy in Romania. He obtained his Masters as a Physician Assistant from Samuel Merritt University in Oakland, CA. Dr. Neagos is fluent in Romanian, and enjoys research and lecturing. He enjoys traveling and has a big interest in Art.

Oganes Akopyan, MD

Dr. Akopyan received his medical degree from the American University of the Caribbean School of Medicine in 2003 and completed his residencies in 2008. Since then, he has been providing medical services at CCHC. Dr. Akopyan is fluent in Armenian and interests include travel and technology.

O.Paronian

Oganes Paronian, MD

Dr. Paronian completed his internship at Queen of Angels-Hollywood Presbyterian Medical Center in 1995. Since, he has worked for various Medical Clinics including: Ter-Zakarian Medical Clinic and Queen’s Care Family Clinic. He is fluent in Armenian, enjoys traveling, and rooting for his favorite team The Los Angeles Lakers.

O.Olanipekun

Olufunmilayo Olanipekun, DPM

Dr. Olanipekun received her medical degree from Temple University School of Podiatric Medicine in 2006. She completed her residency at UCLA-Olive View Medical Center in Los Angeles, CA and has since has attained her Masters of Sciences in Health Services from Western University College of Allied Health.  She is Board Certified in Podiatric Medicine. She practices at both Queens Care Health Center as well as Serra Community Medical Center. In her spare time Dr. Olanipekun enjoys traveling and visiting new countries.

Rachelle Cayago

Rachelle Cayago, PA-C

Ms. Cayago received her medical degree from Western University of Health Sciences in Pomona, CA and completed her rotations at various medical centers including Chino Valley Hospital and Arrowhead Medical Center. Since, she has practiced at clinics including: Tweedy Medical Clinic, Clinica Medica Familiar, and Magnolia Women’s Center. Ms. Cayago is fluent in Tagalog.

Ranjit Shenoy

Ranjit Shenoy, MD

Dr. Shenoy received his medical degree from Armed Forces Medical College in Pune, India. He continued his medical career with an internship at the Asvini Indian Naval Hospital in Bombay, India and a residency at the University of Oklahoma Health Sciences in Oklahoma City, OK. Since, he has practiced at many clinics and hospitals including Kaiser Permanente and is licensed in CA, OK, MO, MS, and NM. While serving in the Indian Peace Keeping Force, Dr. Shenoy was awarded the Distinguished Service Award. His hobbies include exercise and travel.

Rosa Mazariego

Rosa Mazariego, LCSW

Ms. Mazariego received her Master of Social Work from the University of Southern California in 2010. She is fluent in Spanish and her previous experience includes positions held at Community Family Guidance Center, Pacific Clinics, and Hillsides. Ms. Mazariego enjoys various activities, including walking, spending time with family, and watching movies.

Scott Foust

Scott Foust, MD

Dr. Foust received his medical degree from University of Southern California/Keck in 2006 and continued his pediatric residency at Los Angeles County until 2009. Dr. Foust enjoys traveling and has lived abroad for a number of years prior to moving back to California.

Shirley Sotelo

Shirley Sotelo, LCSW

Ms. Sotelo received her Master’s in Social Work from George Mason University in 2010. Work experience includes: Hope Street Family Center at California Hospital Medical Center in Downtown Los Angeles, George Mason University, and Northern Virginia Family Service. She is also fluent in Spanish.  Ms. Sotelo loves animals; she rescued a Jack Russell Terrier named Franny five years ago.

Taline Garabedian

Taline Garabedian, PA-C

Taline graduated from Western Univeristy of Health Sciences and has previously worked for prominent community clinics including Lanark Family Medical Clinic and Central City Community Health Center. Taline is fluent in Spanish and Armenian.

Victoria A

Victoria Alaev, MD

Dr. Alaev received her medical degree from Tadjik State Medical Institute in Russia in 1982. Prior to coming to the United States, she worked as a pediatrician for over 10 years. She completed both her internship and residency at King/Drew Medical Center in 2002. In addition to her work at CCHC, she has evaluated newborns at Providence St. Joseph Medical Center in Burbank for the last 10 years. Dr. Alaev is fluent in Russian.

Wen-Shi Chang

Wen-Shi Chang, MD

Dr. Chang received her medical degree from St. George’s University School of Medicine in 2008 and continued her education with a residency at the University of Medicine & Dentistry of New Jersey. Dr. Chang is fluent in Mandarin and Korean and has a zeal for community involvement.

Zareh A

Zareh Avanessians, RDH

Mr. Avanessians received his dental education at Pasadena City College and is Board Certified as a Registered Dental Hygienist. He has practiced at facilities including Desert Dental, Premier Dental Care, Glendale Premier Dental Center, and Sherman Way Dental Group. He is fluent in English, Farsi, and Armenian. Zareh enjoys archery, photography, and spending time with his family.

CCHC accepts most insurances and payer sources including:

  • Family PACT
  • CHDP

We work with patients to ensure you receive the care you need regardless of ability to pay. We have many programs that help us to provide services with little to no cost and our team can help you determine if you qualify.

Some of these programs are available for services such as family planning, prenatal care, and well child care/immunizations. CCHC also accepts cash payments based on your ability to pay. Ability to pay is calculated based on family size and income per the federal poverty guidelines. If you have questions or wish to apply for a Sliding Fee Scale discount, please speak with a receptionist in the health center or contact the clinic directly.

Please bring your insurance card to the health center at every visit. If you or your family does not have health insurance we can help you apply. Please visit our enrollment program for more information or contact the clinic directly.

If you need to make a payment because you have received a bill, please call (818) 630-6124 or (818) 630-6128. All you need is the patient’s full name, account number (listed on your bill), and a valid credit card with the proper billing address.

Behavioral Health & Social Services

CCHC provides behavioral health and access to social services, understanding that it can play an important role in overall health.

CCHC provides behavioral health and access to social services because this can play an important role in one’s overall health. CCHC has Licensed Clinical Social Workers (LCSWs) available at each site. CCHC’s primary care providers work closely with the behavioral health providers to ensure a coordinated plan of care. Services are intended to meet the patient where they are in their ability to manage their health.

CCHC provides vision services with a comprehensive eye exam and eye disease management that maintains eye health.

Podiatry is the diagnosis and treatment of conditions of the foot, ankle, and lower extremity. Services include:

CCHC offers health education classes at all four health center locations every month. These classes consist of helpful and important information about diabetes, healthy living for adults and kids, and heart health.

Classes are taught by a Health Educator and are informative, interactive, and fun! All classes are offered in English and Spanish. All classes offer a light snack and refreshment, as well as other incentives for attending. CCHC patients are encouraged to bring family and friends along. Health education classes are also open for anyone in the community to attend.

The prenatal program at CCHC combines routine obstetrical (OB) care and other supportive services to expecting mothers.

The prenatal program at CCHC combines routine obstetrical (OB) care and other supportive services to expecting mothers. A unique component to CCHC’s prenatal program is the Comprehensive Perinatal Services Program (CPSP), which helps enhance the patient experience, and helps create healthier moms and babies. In addition to OB care with a CCHC Provider, Prenatal Coordinators offer one-on-one appointments to address patient needs related to nutrition, health education, and psychosocial support.

Each expecting mother goes through a Prenatal Program orientation that will help soon-to-be mothers understand what to expect throughout their care at CCHC. Every trimester, the Prenatal Coordinator will meet with the expecting mother to ask questions and create an individualized care plan. The Prenatal Coordinator will also meet with moms after delivery to see how things are going and to make sure both mom and baby have all of the support they need to be healthy. Additional appointments are available as needed for help with family planning, breastfeeding, and other topics. All CPSP visits with the Prenatal Coordinator are free and moms will even receive a small gift just for coming in.

The family planning program provides individuals with comprehensive family planning and related reproductive and preventive health services.

CCHC is a Title X provider, which is a federal program that provides comprehensive family planning and other related reproductive and sexual health services to low income and uninsured individuals.

Family planning services available at CCHC include:

  • Contraceptive (birth control) services
  • Preventive services
    • screening for breast cancer
    • screening for cervical cancer
  • Pregnancy testing and counseling
  • Screening and treatment of sexually transmitted infections (STI)
  • Screening for human immunodeficiency virus (HIV)
  • Assessment of reproductive health needs

CCHC also offers assistance and services relating to family planning through Family PACT. Family PACT is California’s program to provide comprehensive family planning services to eligible low income men, women, and adolescents for free.

There is no minimum age for enrollment in Family PACT. Please ask the front desk staff for help with the application.

Community Outreach

The community outreach program provides individuals and families with information related to all CCHC services, with the goal of linking the community to needed services.

The Community Outreach team provides educational workshops to community-based organizations on topics related to CCHC services, access to care, family planning, reproductive health, healthy living, and other health related services and programs.

Check Yourself!

Check Yourself! is a CCHC developed campaign funded by the federal Title X family planning program. Check Yourself! is intended to engage adolescents, teens, and young adults about family planning and other related reproductive health issues. The CCHC Community Outreach team partners with middle schools, high schools, community colleges and other community based organizations that provide services to adolescents, teens, and young adults.

The Check Yourself! campaign spreads the word through presentations, distribution of educational material and promotion during community events, health fairs, and through social networking sites.

  • Access to Clinic Services
  • Contraception/Birth Control
  • Sexually Transmitted Infections (STIs)
  • Healthy Relationships
  • Intimacy & Values

As part of the Affordable Care Act (ACA), also known as “ObamaCare,” all US citizens and lawfully present residents of California are required to have health insurance. If not, they will pay a penalty. For non-residents who want health coverage, there is also an option called My Health LA. CCHC can help!

Certified Enrollment Counselors can help you apply to one of Covered California’s qualifying health plans, Medi-Cal, or the My Health LA program.

You will need the following documentation to complete an application:

  • Income information (pay stubs and/or tax forms)
  • A form of ID
  • Proof of citizen or lawful presence (for My Health LA, proof of Los Angeles County residency)
  • Social Security Number (if applicable)
  • Zip code

Below is additional information about Medi-Cal, Covered California, and My Health LA to help you understand the health coverage options available through CCHC.

Medi-Cal

Medi-Cal is an insurance type offered by the State of California. It is for low-income individuals and families. Medi-Cal does not have a monthly premium, but may have some out of pocket costs when using the insurance.

Below are the eligibility requirements for Medi-Cal (these are estimates and should be confirmed during the application process):

  • Household size and income (determined by Modified Adjusted Gross Income that was used on your taxes)
  • Adults under 65 with income less than 138% of the federal poverty line (FPL)
  • Children under 19 with income less than 266% FPL
  • Pregnant women with income less than 213% FPL
  • Immigrants with Satisfactory Immigration Status (SIS) who also meet the above requirements (SIS: non-citizens who are lawfully present with permanent resident card, work visa, etc.)

Covered CA is the state’s insurance marketplace. Covered CA offers affordable and low cost health insurance options. Based on your income, you (and your family) may qualify for assistance through Covered CA. Some of the assistance, also known as a subsidy, includes assistance paying your monthly premium and out-of-pocket costs.

My Health LA

My Health LA is a no cost health care program for people who live in Los Angeles County and meet the eligibility requirements. It is a free health care program for individuals and families who do not have, and cannot get, health insurance. CCHC is a My Health LA Community Partner Clinic.

Eligibility requirements include:

  • People who live in Los Angeles County
  • Age 6 and older
  • Have an income at or below 138% of the federal poverty line
  • My Health LA does not require individuals to have legal immigration status
