At CCHC, we are dedicated to ensuring you receive the care you need regardless of ability to pay, race, religion, age, gender or sexual orientation. We have many programs that help us to provide services at little or no cost to you, and our team can help you determine if you qualify. CCHC also accepts cash payments based on your ability to pay. We can offer a wide variety of services through a Sliding Fee Discount Program. The Sliding Fee Discount Program is available to all eligible patients and is calculated based on annual household income and family size per the federal poverty guidelines.

Good Faith Estimate
As part of the No Surprises Billing Act, uninsured and self-pay patients are eligible to receive an estimate of the possible changes for the appointment(s) that you schedule with CCHC. Your financial category will depend on your income and the number of people in your household. The estimate will contain additional information on how to determine your self-pay category.
