At CCHC, it is a top priority for all of our patients to have access to care, information and resources that address health concerns, and provide additional support when receiving care at our facilities. These different types of support include, but are not limited to:

Translation – CCHC has services available to help us communicate with our patients who are in need of assistance. We strive to ensure courteous, consistent and efficient communication that is appropriate for everyone whenever possible. In addition to some of our bilingual staff that we have at our clinic sites, our teams can also utilize interpreter services, which may be available through your insurance company, and/or through other agencies CCHC has partnered with. Please let us know if there is any communication assistance required at your next visit, and our team will be pleased to coordinate..

Coverage for Emergencies After Hours –  It is a top priority for us as an organization that everyone remains safe. This is why we have processes in place after office hours to make sure patients remain as safe as possible. All our staff are versed and trained on what to do in the event of any sort of emergency, including medical emergencies. We also want all our patients to have access to care, information, and resources that address health concerns and problems after regular operating hours in different languages and that fits a wide variety of comprehension. These numbers will connect you with licensed professionals after regular office hours to address medical emergencies.

In the event someone is ever experiencing a medical emergency, please dial 911 immediately or visit an emergency room or emergency department.

Transportation Services – Our goal at CCHC is to ensure that transportation is accessible to all of our patients in need. We want to make sure that getting to us, traveling between sites, and other relevant appointments have minimal barriers whenever possible. We have identified several options which include, potentially taking advantage of our free transportation services, coordinating transportation through your insurance carrier, and/or potentially ride sharing services and applications. Visit Assistance Services

Visit assistance services are available on site, at each of our locations. We offer mobility impairment tables, and hearing devices to those in need. If you require such assistance, please notify our staff before your appointment, and we will ensure that your request is accommodated for.

Eligibility Workers

If you are uninsured or need public assistance, our eligibility workers are here to help you and your family address those concerns. They will assist in applying for the best healthcare coverage for you and your family, based on household size and income. Click here for more information.
